Travel Form at the Beginning or at the End of Employees Contract

1. This Form must be completed by the employee and sent to the HR Office which will confirm according to the President’s approval according to the terms and conditions of each employee’s contract. It will then be sent to for processing.

2. The employee should notify the HR of his/her travel requirements at least three weeks before the travel dates.

3. A copy of the employee’s passport must be attached to this form.

4. Copies of the accompanied family members’ passports must be attached, also.
